What you'll do

• Conduct comprehensive nursing assessments in clients’ homes and collect OASIS information
• develop, implement, and update individualized care plans
• administer prescribed medications, treatments, and skilled nursing procedures
• monitor health and report changes
• educate clients and families
• collaborate with caregivers, aides, and interdisciplinary team members
• maintain timely documentation
• respond to emergencies
• teach home safety practices.
