The Clinical Support Supervisor — Utah LIFT Program role is hybrid, based in the Vernal / Uintah Basin area of Utah, with regional travel to partner FQHC clinic sites as needed. Compensation is not specified. Activate Care’s mission is to improve health equity and health outcomes through care coordination for people with health-related social needs. The supervisor provides clinical guidance and escalation support to Community Health Workers serving participants with chronic conditions. The role strengthens safe, evidence-based support and coordination between CHWs and participating clinics while maintaining clear boundaries between non-clinical CHW services and licensed clinical care.

What you'll do

• Provide structured case consultation to CHWs when needs exceed routine non-clinical support
• reinforce appropriate coaching and clinical escalation boundaries
• advise on common chronic conditions including diabetes, hypertension, obesity, COPD, asthma, and cardiovascular risk
• support standardized education, telehealth, appointment, and escalation workflows
• coach CHWs in motivational interviewing and goal setting
• develop escalation protocols with FQHCs
• guide escalation of symptoms, medication concerns, abnormal information, and safety concerns
• review safety events and near misses
• coordinate with FQHC clinical teams
• support CHW training and case-based materials
• review program trends and quality measures
• maintain HIPAA, privacy, security, and documentation compliance.

Why this role

Requires an active, unrestricted Utah RN or LPN license, relevant chronic-condition clinical experience, and care-management or similar program experience. Local travel and a valid driver’s license with reliable personal transportation are required. Work visa sponsorship is not offered.
