What you'll do
- Complete initial patient and family assessments
- develop care plans with nursing diagnoses and palliative actions
- coordinate with the clinical team and supervise LPNs and hospice aides who may support patients
- assist with pain management and support
- modify and reevaluate care plans
- monitor, record, and report symptoms and changes
- obtain physician orders as required
- participate in on-call duties under the on-call policy.
