What you'll do

• Respond to referrals and crisis calls, including on-call responses
• meet immediate needs
• provide field-based advocacy, case management, and emotional support
• assess needs and develop service plans
• coordinate referrals and transportation
• collaborate on safety plans and multidisciplinary meetings
• document interactions
• support families when appropriate
• complete required reports and training.
