Registered Nurse – Mental Health Case Manager

The full-time schedule is Monday through Friday, with eight working hours scheduled between 7:00 AM and 5:00 PM, a one-hour unpaid lunch, no weekends, no on-call requirements, and federal holidays off. GiaCare supports military healthcare facilities and the military community. The nurse coordinates individualized care across inpatient, outpatient, community, and telephonic settings to support continuity of care and patient outcomes. The role connects patients, caregivers, clinicians, and community resources while maintaining care plans and documentation.

What you'll do

• Assess patient needs and develop, implement, coordinate, and monitor individualized care plans
• provide case management across inpatient, outpatient, onsite, community, and telephonic settings
• coordinate preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions
• support discharge follow-up and continuity of care
• integrate nursing case management with utilization management and social-work case management
• collaborate with healthcare professionals, caregivers, and community organizations
• identify healthcare and community resources
• maintain patient records and case-management documentation
• provide health education
• contribute to assessment tools, clinical guidelines, processes, and resource databases
• participate in quality improvement, meetings, and training.
