Blended Case Manager

What you'll do

• Manage a caseload of 30 consumers and meet them according to level-of-care requirements
• assess needs and develop service plans with consumers
• link consumers to services and community resources
• monitor service delivery
• help resolve service problems and advocate when needed
• build informal support networks with family involvement when permitted
• complete required documentation
• attend supervision and required training
• provide crisis prevention and intervention as primary contact and personal support, including 24/7 on-call services.
