What you'll do
- Oversee patient care from admission to discharge
- review and approve clinical documentation, including OASIS data, plans of care, and visit notes
- lead interdisciplinary case conferences
- guide field staff on complex cases
- monitor patient progress and adjust care plans
- supervise, mentor, and manage field clinicians
- conduct field supervisory visits
- support recruitment and orientation
- facilitate training
- participate in QAPI, audits, corrective actions, and regulatory surveys
- collaborate with agency leadership, intake, and authorization teams
- address patient and family concerns
- assign staff to caseloads.
