What you'll do

• Assess medication safety against the patient’s active medication list and administration history
• identify medication reconciliation gaps, IV compatibility issues, and adverse-reaction patterns
• evaluate whether the clinical picture is complete
• distinguish nursing-scope issues from those requiring pharmacy or prescriber escalation
• verify that findings are carried through with appropriate dose holds, notifications, and chart documentation.
