Job overview
The Home Health Certified Nursing Aide (CNA) role is onsite in Reston, VA, with pay of $18–$20 per hour. Pathwell Health provides individualized care based on a care plan set by an LPN, in clients’ homes or care facilities. The aide supports clients’ daily living, personal care, companionship, and socialization while helping maintain a safe environment. The role also encourages self-care, documents care, and reports changes in health status to support continuity of care.
What you'll do
- Provide routine individualized healthcare based on an LPN-set care plan
- monitor or report changes in health status
- assist with bathing, dressing, grooming, and activities of daily living
- provide companionship
- escort clients to recreational activities and medical appointments
- perform light housekeeping as needed
- encourage self-care
- maintain accurate care documentation and confidentiality
- and perform other assigned duties.
