What you'll do

• Conduct initial screenings, crisis stabilization, and diversion assessments
• develop diversion plans with clients
• mediate housing-related barriers
• coordinate with community partners and county financial assistance workers
• verify eligibility and process applicable payments
• maintain accurate HMIS data and referral resources
• provide education, coaching, and advocacy
• contribute to program evaluation and team meetings
• represent the organization at assigned community events.
