What you'll do

• Provide direct care and daily living support
• assist with hygiene, grooming, meals, and household tasks
• implement individualized support plans and document progress
• accompany clients to appointments and community activities
• administer medications and monitor health conditions according to care plans
• encourage skill-building, independence, and community integration
• communicate with supervisors, family members, and healthcare professionals.
