What you'll do
- Participates in interdisciplinary rounds and case conferences.
- Manages a Behavioral Health Care Management caseload for members with complex needs.
- Serves as a Health Home and HARP liaison.
- Provides clinical consultation to Care Management staff.
- Conducts outreach and follow-up activities related to quality initiatives.
- Collaborates with various teams to enhance clinical workflows and monitor regulatory requirements.
