What you'll do

• Use lived behavioral health experience to conduct outreach and engagement
• collaborate on referrals, resources, and interventions with a multidisciplinary team
• screen referrals for OnTrack NY eligibility
• connect people who are ineligible to other appropriate services
• complete clinical documentation in the electronic medical record
• coordinate community resources
• attend supervision and team meetings
• complete training and maintain de-escalation proficiency
• provide additional clinic support as needed.
