What you'll do

• Develop relationships with hospitals, physicians, skilled nursing and assisted living communities, case managers, and referral sources
• promote Home Health and Hospice services
• review referrals and assess eligibility
• perform bedside and community assessments as needed
• educate patients and families and obtain consents
• coordinate admissions
• obtain physician orders and documentation
• maintain EMR records
• resolve referral barriers
• and comply with applicable regulations and agency policies.
