Community Care Relations & Field Support Specialist

The Community Care Relations & Field Support Specialist works onsite in Morris, IL, supporting expansion across Grundy and LaSalle Counties. Pay is $20–$25 per hour. Complete Health Services' mission is to help seniors with chronic illnesses live with greater independence, dignity, and support through nurse-practitioner-led care. The specialist builds community relationships and supports caregivers, clients, and service operations, helping new clients transition into care and ensuring a high-quality experience as the local team grows.

What you'll do

• Attend networking events, community fairs, and senior resource meetings
• assist with caregiver hiring, interviewing, and onboarding
• support clients and caregivers during the first weeks of service
• help with scheduling, documentation, and communication
• meet new clients at home for intake assessments and service starts when needed
• and participate in the on-call rotation one week every 4–6 weeks.
