Entitlements Case Manager

The Entitlements Case Manager role is based in Baltimore, MD, serving clients primarily in community-based group homes and approved community or office locations; telehealth may be used under stated site conditions. Lighthouse Behavioral Health Center and Community Wellness Outpatient Mental Health Center provide community-based behavioral health services, including case management. The case manager helps clients apply for and maintain benefits and stabilizing resources while coordinating care across behavioral health programs. Through individual and group sessions, assessments, documentation, and referrals, the role supports client access to services and program goals while meeting regulatory and clinical requirements.

What you'll do

• Assist clients with applications for benefits and entitlements
• facilitate individual and group case management and intake orientation sessions
• offer make-up sessions for missed appointments
• complete assessments, reassessments, plan reviews, and daily documentation
• coordinate with program staff and external providers
• address social determinants of health and provide crisis or safety interventions as needed
• follow COMAR, CARF, Medicaid, licensing, and ethical requirements
• participate in audits, supervision, and quality reviews.

Why this role
