The role exists to provide supportive counseling and care coordination for residents and their families. The Social Worker serves as a key member of the interdisciplinary team, assisting with social, emotional, and economic concerns to help patients achieve an optimal level of functioning.

What you'll do

• Provides supportive services and counseling on healthcare programs.
• Coordinates care among patients, families, and healthcare teams.
• Responds to suspected abuse or neglect.
• Develops plans of care and provides referrals to community resources.
• Participates in care conferences and transitional rounding.
