What you'll do
- Assess clients’ substance-use needs and treatment goals
- provide counseling, case management, groups, crisis intervention, advocacy, psychoeducation, and follow-up
- develop individualized intervention plans
- coordinate with internal staff and external providers
- document services in the electronic medical record
- arrange or escort clients to treatment appointments when needed
- maintain a flexible schedule that includes evenings, weekends, and holidays.
