Registered Nurse (RN) – Rehabilitation and Memory Care

The Registered Nurse (RN) – Rehabilitation and Memory Care role is onsite in Toledo, OH. Compensation is $39–$43 per hour, with day shifts and 12-hour schedules. Merit House Rehab provides nursing care in a rehabilitation setting and describes a team focused on patient-centered care. The nurse also supports patients with memory impairments and disabilities and collaborates with interdisciplinary teams on education, case management, and discharge planning.

What you'll do

• Perform patient assessments, take vital signs, and conduct physical examinations
• develop care plans
• administer medications, injections, and treatments
• document in EMR/EHR systems while following HIPAA requirements
• provide care for patients with dementia, Alzheimer’s disease, memory impairments, or developmental disabilities
• assist with diagnostic evaluations, specimen collection, catheterization, and wound suturing using aseptic techniques
• collaborate on discharge planning, case management, and patient education.
