What you'll do

• Participate in planning and evaluating individual service plans
• train and assist residents with daily living activities and document interactions
• accompany residents on outings and travel training
• provide counseling, behavioral intervention, and crisis management
• support daily residence management
• respond to medical or psychiatric emergencies
• provide transportation as needed
• write daily progress notes
• monitor medication management
• maintain cleanliness
• advocate for resident benefits
• teach independent living skills
• coordinate with local service providers and support services.
