The Veterans Case Manager is a full-time, onsite role in Fort Belknap Agency, MT. The posting lists wages of $23.32–$28.43 per hour, DOE. The Great Plains Veterans Services Center’s Veterans Support Services program helps veterans access earned benefits, employment support, housing assistance, wellness resources, and case management. The case manager serves as a local point of contact, assessing needs and coordinating individualized support for veterans in the Fort Belknap service area. Through outreach, advocacy, referrals, follow-up, and accurate documentation, the role helps veterans connect with benefits and community resources and work toward their service goals.

What you'll do

• Conduct outreach, home visits, intake, and needs assessments
• develop, monitor, and update individualized service plans
• assist veterans with military documents, benefits, and enrollment
• coordinate referrals and follow-up with VA, tribal, state, local, and community resources
• advocate and liaise with veterans, families, providers, and partner agencies
• support screening, safety actions, workshops, and outreach events
• maintain complete and confidential case records.

Why this role
