The Patient Care Coordinator role is onsite in Waco, TX, at $18 per hour, Monday through Friday, 8:00 a.m. to 5:00 p.m. CST. Ennoble Care provides mobile primary care, palliative care, and hospice services, including programs for chronic care and remote patient monitoring. The coordinator supports continuity of care by organizing patient communications, provider schedules, referrals, follow-up, and documentation. The role connects patients, families, providers, and clinical staff so care needs and barriers are addressed promptly.

What you'll do

• Coordinate patient care activities in the EMR
• assist with referrals, laboratory orders and follow-up, medication refill requests, and durable medical equipment coordination
• communicate with patients, providers, families, and interdisciplinary teams
• document interactions and promptly communicate condition changes or urgent matters
• use office technology and support daily operations.
