What you'll do

• Develop referral relationships in Algonquin and surrounding communities
• connect with hospitals, rehabilitation facilities, senior living communities, hospice and home health agencies, physician offices, and other organizations
• conduct in-person outreach, follow-ups, presentations, and networking
• generate qualified referrals and help convert opportunities into clients
• meet prospective clients and families to discuss care needs and services
• represent the organization at community events
• track outreach, referrals, follow-ups, and results
• collaborate with the office team
• assist with local marketing and social media as needed.
