Care Coordinator, Embedded is an onsite, full-time role at one or more partnered long-term care facilities in the Alexandria, VA area. Compensation is not specified. Ennoble Care provides mobile primary, palliative, and hospice care, including support for patients with chronic conditions and limited mobility. The Coordinator works alongside providers and facility staff to maintain continuity of care, support care plans and transitions, communicate patient needs, and document coordination activities for a dedicated group of residents.

What you'll do

• Coordinate care for long-term care residents using the company EMR
• engage onsite with residents regarding care plans, follow-ups, and transitions
• collaborate with providers
• serve as liaison among providers, facility staff, residents, and families
• communicate changes and urgent concerns to the clinical team
• document interactions
• and follow workflows and escalation protocols.

Why this role

The schedule is Monday through Friday, 8:00 AM–4:30 PM EST or around facility hours, with no weekends and no on-call. An active clinical certification or license and at least one year of patient-facing experience in long-term care, skilled nursing, or a similar setting are required. Travel between facilities may be required.
