What you'll do

• Coordinate and monitor resident care conferences
• create, maintain, edit, and audit care plans
• coordinate ancillary and contract services
• complete initial assessments and interviews
• support residents, responsible parties, and families through placement and crisis situations
• protect resident health information
• complete timely documentation
• coordinate personal needs and advocate for residents
• use community resources
• guide and train staff as needed.
