The role contributes to resident care and daily functioning by providing personal care, monitoring and documenting health indicators, supporting mobility and nutrition, and helping residents maintain dignity, choice, and independence in accordance with care plans and facility procedures.

What you'll do

• Take and record vital signs, height, weight, and fluid intake and output
• make routine rounds every two hours
• assist with bathing, grooming, dressing, toileting, eating, and hydration
• transfer, position, and turn residents
• observe and report changes in condition or skin
• contribute to assessments and care plans
• provide restorative and dementia-related support
• document care
• protect resident rights and confidential information
• follow infection-control and safety procedures.
