Traveller Social Worker, Acute Medical Inpatient

What you'll do

• Participate in discharge planning, social care plans, and resident assessments
• complete medical forms, reports, evaluations, charting, and transportation arrangements
• participate in interdisciplinary meetings
• support safe discharge and placement
• communicate with patients and responsible persons
• provide information about Medicare, Medicaid, and financial assistance
• consult with staff on psychosocial concerns and placement
• participate in quality programs
• maintain confidentiality and follow HIPAA requirements.
