What you'll do

• Build and maintain relationships with community organizations and referral sources
• conduct outreach and attend community events
• explain AFC services and general eligibility
• identify potential referrals and collect appropriate contact information
• follow up with contacts
• connect interested people with AFC staff for screening, assessment, and enrollment
• maintain outreach and referral records
• track engagement and referral sources
• coordinate handoffs with the AFC team.
