Hospice Chaplain and Bereavement Coordinator

What you'll do

• Conduct spiritual assessments and develop individualized spiritual care plans
• provide pastoral counseling and spiritual support
• coordinate with community clergy when requested
• participate in interdisciplinary meetings
• oversee bereavement risk assessments, plans, grief support, memorial activities, and post-death follow-up for a minimum of 13 months
• document services and coordinate referrals as appropriate.
