Mental Health Support Specialist

What you'll do

• Assess client risk and needs
• provide direct-care services and activities of daily living support
• coordinate care with service providers
• prepare individualized service plans and treatment updates
• recommend and coordinate services supporting community independence and stability
• respond to crisis situations with 24-hour availability
• complete progress reports within 24 hours
• collaborate with physicians, therapists, psychiatrists, case managers, and substance abuse providers
• monitor and document progress
• participate in supervision and training.
