What you'll do

• Assess patient needs, urgency, health history, vital signs, risk factors, and disease conditions
• obtain consent and protect confidentiality
• provide and coordinate care
• administer prescribed medications
• collaborate on patient goals and monitor progress
• educate patients and families
• serve as liaison between medical and behavioral health staff
• document care in the EHR
• participate in multidisciplinary teams and support quality and regulatory compliance.
