What you'll do

• Conduct comprehensive patient assessments and develop individualized care plans
• provide skilled nursing care under physician orders and regulatory standards
• monitor progress and adjust care plans
• coordinate with physicians, therapists, aides, and interdisciplinary team members
• review clinical documentation for accuracy, completeness, and compliance
• educate patients and families about care plans, medications, and disease management
• support clinical staff onboarding, training, and performance development
• prepare for audits, surveys, and regulatory reviews
• maintain compliance with state and federal home-health regulations
• communicate updates to the Clinical Manager and Administrator.
