The position supports Syracuse Community Health’s integrated behavioral health and primary-care model at a mission-driven Federally Qualified Health Center. The counselor provides individual, family, and group counseling, assesses behavioral health needs, develops treatment plans, and monitors progress. Collaboration with primary-care and psychiatric providers supports integrated, patient-centered care, while crisis intervention and advocacy address patients’ behavioral health needs.
What you'll do
- Provide individual, family, and group counseling
- complete behavioral health assessments and diagnoses
- develop and adjust individualized treatment plans based on outcomes
- collaborate with primary-care providers, psychiatric providers, and interdisciplinary team members
- provide crisis intervention, patient advocacy, and behavioral health education
- document in the electronic medical record.
