What you'll do

• Develop and update individualized client care plans
• conduct supervisory RN visits to assess client condition and service quality
• provide hands-on caregiver training
• verify caregiver competency with delegated tasks
• reassess client needs and satisfaction every 90 days or as needed
• document condition changes and adjust care plans
• communicate with clients and designated family members
• report conditions requiring further attention to the Agency Director.
