What you'll do

• Conduct direct care rounds on the assigned unit
• assist residents with activities of daily living
• maintain positive relationships with residents and patients
• notify the charge nurse of changes in condition
• obtain vital signs, weights, and heights as instructed
• complete assigned duties and documentation
• coordinate and prepare for admissions, transfers, and discharges in the Transitional Care Unit
• maintain a safe and clean work area
• follow infection prevention and control practices.
