What you'll do

• Build and maintain referral relationships with healthcare providers, social workers, and senior living communities
• develop territory sales plans using referral and market-share data
• track activity in CRM tools
• educate partners about home healthcare and hospice services
• coordinate with Nurse Navigators, marketing, and clinical teams
• manage accounts and resolve challenges
• monitor healthcare operations, legal guidelines, competition, and market trends.
