Intensive Case Manager I

The Intensive Case Manager I role is onsite in La Verne, California. Compensation is $24–$27 per hour. The position is part of A Community of Friends’ permanent supportive housing case management team and Intensive Case Management Services. The case manager provides individualized support to formerly homeless individuals and families with chronic medical conditions and co-occurring disorders. The role helps tenants pursue goals, maintain housing, increase income, and build community engagement through assessment, referrals, supportive services, and documented follow-up.

What you'll do

• Provide individual case management, referrals, and supportive services
• create group services
• maintain case notes in CHAMP and HMIS
• process housing applications and interview prospective tenants
• provide crisis management to support housing retention, income, and community engagement
• cover occasional evening and weekend program hours
• report suspected abuse
• present case reviews to the supervisor.
