What you'll do
- Build relationships with patients
- meet with patients and enter data into company software
- call patients to schedule visits and medication pickup
- order and schedule procedures, tests, and screenings
- track care gaps
- maintain interaction records and communicate with providers through electronic health records
- educate patients about healthcare options, insurance benefits, and common conditions
- connect patients with community resources
- coordinate follow-up after hospital discharge.
