What you'll do

• Evaluate patients and develop individualized treatment plans.
• Provide therapy to improve mobility, strength, and functional independence.
• Treat acute conditions including stroke, trauma, cardiac, and post-op cases.
• Monitor patient progress and update care plans.
• Educate patients and families on recovery and home exercise programs.
• Collaborate with interdisciplinary healthcare teams.
• Maintain accurate documentation using EPIC system.
