What you'll do

• Help patients access health insurance, food, housing, transportation, care, and health information
• support culturally diverse and underserved populations
• coordinate patient transportation
• connect patients with health and social-service resources
• conduct outreach to migrant populations
• explain health-center services
• help patients set health-management goals and follow care plans
• coach patients on chronic-condition management and self-care
• assist with appointments and applications
• coordinate providers
• support prescription assistance and 340B pharmacy access
• provide referrals
• and document assistance.
