Compensation is $16–$0 per hour. The Home Health Field Registered Nurse role is onsite in Southfield, MI, working in the agency office and patient homes and traveling between homes. Hope At Home Health Care provides Medicare-certified skilled services to help people receive care in the comfort of home. The nurse evaluates patient needs, develops and updates plans of care, provides specialized nursing services, and coordinates with patients, families, physicians, and other clinicians. The role supports safe, coordinated home care and documents assessments and progress in accordance with agency time frames.

What you'll do

• Conduct initial and ongoing nursing evaluations
• initiate and revise plans of care
• provide specialized nursing and preventive or rehabilitative procedures
• prepare clinical and progress notes
• coordinate services and communicate changes in patient condition
• counsel patients and families
• process orders and complete OASIS assessments
• and supervise or teach nursing personnel. Assigned on-call duty may include nights, weekends, and holidays.
