• Conduct psychosocial assessments of emotional, social, and environmental factors affecting patients and families
• provide emotional support and counseling related to end-of-life issues
• coordinate care with the interdisciplinary group or team
• assist families with grief and loss
• connect families with community resources and services
• document social work services according to hospice policies and regulatory requirements.
