What you'll do

• Provide medication reconciliation, dose optimization, adherence support, and drug-level assessments
• partner with physicians and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement
• monitor therapy effectiveness and safety and intervene as appropriate
• support quality-improvement and REMS initiatives
• assist with complex prior authorizations, workflow improvement, patient education, and clinical development.
