Community Outreach Worker

The Community Outreach Worker role is an onsite, full-time position in New York, NY, with compensation of $26.25 per hour. La Casa de Salud works to improve the health and well-being of the South Bronx through integrated, bilingual, culturally competent care for community members. The worker engages people facing homelessness, health disparities, substance use, and mental health challenges, then helps connect them with medical, behavioral health, housing, and social services. The role also operates and maintains a mobile outreach unit and documents outreach, screenings, referrals, and follow-up to support coordinated community-based care.

What you'll do

• Conduct neighborhood outreach and build rapport using trauma-informed, culturally responsive approaches
• provide service information
• conduct screenings and assessments
• assist with telehealth access, referrals, scheduling, and follow-up
• accompany or transport clients as needed
• operate the mobile outreach vehicle and maintain safety logs
• stock and set up outreach supplies
• document activities and collaborate with internal departments and community partners.
