Residential Specialist II, Scattered Site Outreach Program

What you'll do

• Document activities, communications, and encounters
• coordinate with therapists, referral sources, and community agencies
• design, deliver, and document Individualized Recovery Plans
• teach activities of daily living and basic life skills in individuals’ residences
• refer individuals to community supports and resources
• maintain confidential charts and report concerns to supervisors
• travel locally and within the state and may transport individuals in a personal vehicle.
