Adolescent Care Coordinator

The Adolescent Care Coordinator role is onsite in Mount Juliet, TN, with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. Salary is $55,000–$60,000 per year, based on experience. The role supports Embrace U, an outpatient behavioral health program combining intensive outpatient and partial hospitalization services for patients ages 10–18. The coordinator provides administrative and clerical support to program staff while assisting adolescent clients with care needs. The role contributes to program operations through intake and discharge support, client monitoring, psychoeducational groups, care coordination, documentation, and compliance-related tasks.

What you'll do

• Assist with inquiries, intake, admissions, consents, and discharge planning
• provide clerical support and maintain medical records
• assist clients with care needs, take vital signs as directed, monitor clients during PHP time, and perform groups during IOP time
• assist the nurse practitioner with intakes and follow-ups as needed
• participate in treatment team meetings and care coordination
• complete census reports, billing functions, insurance verification, and concurrent reviews as directed
• track forms and supplies and maintain refrigerator logs.

Why this role
