What you'll do

• Provide care for patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes.
• Engage with hospitalists for hospitalized patients.
• Perform assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
• Lead care team for patients able to come to the office.
• Engage with transitional care team for patients unable to come to the office.
• Actively participate in center management and recruiting.
• Supervise Advanced Practice Practitioners.
• Perform other duties as assigned.

What we're looking for

Education

MD or DO

Experience

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required. Must be able to obtain a State Medical License. Board certification or eligibility preferred. Basic Life Support (BLS) certification required within 90 days.
