What you'll do

• Create individual service plans
• facilitate community reintegration activities
• explain community services and assist with eligibility and applications
• provide low-threshold interventions and coordinate detoxification services
• maintain knowledge of trauma-informed care, overdose prevention, medication for opioid use disorder, and community resources
• provide linguistic support and link clients to appropriate services
• implement strategies to meet program performance metrics.
