What you'll do

• Monitor resident care quality
• conduct resident evaluations and interviews
• develop and update individualized care plans
• facilitate interdisciplinary meetings
• complete MDS assessments and documentation on schedule
• maintain compliance with Medicare and skilled nursing regulations
• train staff on MDS, RAPS, triggers, and care planning
• collaborate with nursing leadership and departments
• participate in management meetings and maintain professional relationships with providers and community partners.
