Registered Nurse Case Manager — Hospice

The Registered Nurse Case Manager — Hospice role is field-based in Sheboygan, WI, with a Monday–Friday schedule from 8:30 a.m. to 5 p.m. Pay is $75,000–$90,000 per year. Moments Hospice provides comfort, dignity, and support to patients and families during end-of-life care. The nurse manages a hospice caseload by assessing patient needs, developing individualized care plans, providing skilled nursing and symptom management, and educating caregivers. The role coordinates care with families, physicians, and the interdisciplinary team to support patient comfort and continuity of care.

What you'll do

• Complete initial and ongoing assessments of physical, emotional, and psychosocial needs
• develop and update individualized care plans with patients and families
• provide skilled nursing, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as liaison among patients, families, physicians, and the care team
• and participate in interdisciplinary group meetings.
